Parents and guardians submitting an application for a residence permit for a child or ward through the Polish MOS system must first create a sub-account for the child. After that, they can fill out and send the application online. InPoland reports about this here.
The submission takes place through the MOS portal – Case Management Module. The system has been operational since April 27, 2026, and applications for various residence permits can be submitted online by foreigners. How to create a child’s sub-profile in MOS
To submit an application on behalf of a child or a person under guardianship, the legal representative must first create a sub-account for them. To do this:

Log into the MOS portal.

Open the section «My Account» → «Personal Information» (Moje konto → Dane osobowe).

Next to the section «Children/Ward Accounts» select «Add Account» (Dodaj konto).

After creating the sub-profile, fill out and submit the application for the child or ward.
The sub-profile is needed to submit the application specifically for the child or ward, and not for the legal representative. We previously discussed more about new submission rules through MOS.
What payments need to be added to the application
The application must include confirmation of payment for the stamp duty and the issuance of the residence card. According to InPoland, the amount of the stamp duty depends on the type of permit and ranges from 340 to 640 zlotys. An additional 100 zlotys is required for the issuance of the card.
Payments for the fees cannot be made directly through MOS. Therefore, the payment transfer confirmation should be added to the application as a separate file: this can be an electronic payment confirmation, a scan, or a clear photo of the document.
