Advancing Medical Evacuation Drones in Ukraine
Ukraine is at the forefront of developing heavy-duty unmanned aerial vehicles (UAVs) designed to evacuate critically wounded soldiers when ground transport is impossible. In a groundbreaking 2024 mission, Ukrainian forces completed the world’s inaugural medical evacuation using a heavy drone, marking a major breakthrough in battlefield medical technology. Specifically, in October 2026, the Ukrainian Armed Forces successfully extracted an infantryman from the 30th Separate Mechanized Brigade using the hexacopter "Heavy Shot Ayudag-6." This drone weighs 98 kg, can carry up to 130 kg of cargo, and fly more than 15 km one way.
Preparation for this mission spanned several months and included valuable operational insights from the 92nd Separate Mechanized Brigade’s reconnaissance company. To ensure mission safety, allied units deployed bombers to distract enemy anti-air drones, highlighting a comprehensive strategy to protect the evacuation team. The Brave1 Defense Innovation Cluster, supported by Ukraine’s Ministry of Defense, actively drives the development of these life-saving UAVs.
Cutting-Edge Technology and Innovation in Medical Drone Evacuations
In 2024, Brave1, in collaboration with military experts, established specific requirements for evacuation drones, including:
- Capability to carry 130 kg over distances of at least 15 km;
- Integrated communications systems featuring at least three alternative channels.
These UAVs are equipped with lidar sensors, optical stabilization systems, and navigation technologies that function without relying on GPS.
One notable example is the "Stargate" drone, created by a Ukrainian company within the Brave1 cluster. Tailored for rapid deployment upon assault unit requests, Stargate can be ready within 10 minutes, carry up to 150 kg at speeds up to 105 km/h, and includes a medical module with stretchers. Its AI-driven software allows the drone to autonomously plot routes and return safely, even under complete navigation signal jamming.
Within just one week, assault troops from the CODE 9.2 regiment executed two aerial evacuations from the gray zone—areas inaccessible by land due to rugged terrain, water obstacles, and threats from enemy FPV drones. The drone covered nearly 30 km one way, successfully returning with wounded personnel while overcoming hostile electronic warfare through AI-based navigation. Heavy strike drones provided cover by suppressing enemy positions, ensuring mission security throughout.
The advancement of drone-based casualty evacuation is revolutionizing battlefield medical care. These innovations not only reduce risks for medical teams but also accelerate the delivery of injured soldiers to hospitals, especially amid ongoing combat. This progress underscores the Ukrainian Armed Forces’ commitment to leveraging state-of-the-art technology to enhance operational effectiveness and save lives on the front lines.
In addition to the remarkable achievements of the "Heavy Shot Ayudag-6," the ongoing advancements in medical drone technology are exemplified by the successful airlift of two critically injured soldiers by the StarGate drone, which underscores the commitment to enhancing battlefield medical support. This operation has prompted plans for establishing a dedicated medical aviation unit, further emphasizing the strategic importance of UAVs in modern warfare.
