Wildfire Smoke Threatens Heart and Brain, Scientists Warn

Lungs Aren’t the Only Target

Wildfire smoke doesn't just damage the lungs—it also harms the heart and brain. Long-term exposure to polluted air increases the risk of stroke, cardiovascular disease, preterm birth, and cancer. Research indicates that there is no safe level of chronic smoke exposure.

Wildfire smoke contains fine particulate matter PM2.5, nitrogen dioxide, ozone, aromatic hydrocarbons, and lead. The smallest PM2.5 particles can penetrate deep into the lungs and enter the bloodstream. Prolonged exposure to PM2.5 from wildfire smoke raises the risk of stroke in older adults. Additionally, the smoke triggers oxidative stress in the body. According to Yang Liu, an environmental health researcher at Emory University,

“smoke is not just a respiratory hazard. It triggers oxidative stress in the body, which can accelerate the development of various diseases.”

How Smoke Impacts Health

Air contaminated by smoke has been linked to a rise in heart attacks and worsening mental health. When wildfires spread into populated areas, plastics, heavy metals, arsenic, and other toxic substances are released into the air. PM2.5 particles are associated with the development of chronic and autoimmune diseases. During pregnancy, smoke exposure increases the likelihood of preterm birth and low birth weight.

Since the start of 2026, more than 1.5 million hectares have burned in the United States—more than double the average annual area burned three decades ago. A 2026 study in Science Advances estimates that between 2006 and 2020, long-term smoke exposure in the U.S. may have been linked to over 24,100 deaths per year. Another study in Nature (2025) projects that between 2026 and 2055, smoke containing PM2.5 could cause roughly 1.9 million premature deaths.

After several days in smoky air, symptoms such as coughing, sore throat, chest tightness, headache, and difficulty concentrating may persist. Some studies indicate a connection between PM2.5 exposure, cognitive impairment, and depression. Overall, the health consequences of wildfire smoke are severe and demand attention from both society and the medical community.

These studies highlight the importance of understanding how wildfires impact not only the environment but also human health. As wildfires become more frequent due to climate change and human activity, steps must be taken to reduce air pollution and its effects on the population. Furthermore, government and health organizations play a critical role in developing strategies to respond to smoke-related health risks, which could prove essential for protecting public health.
