Research presented at the European Society of Cardiology Congress ESC 2026 has found a possible connection between high blood xylitol and a higher chance of serious cardiovascular problems. The findings are based on data from 17,710 people enrolled in two observational studies, one in Canada and one in Europe. Investigators observed that elevated levels of this sugar alcohol were associated with more cases of heart attack, stroke, and early death. Xylitol is a common low-calorie sweetener used in many sugar-free products, making these findings particularly relevant to everyday consumers.

What the Research Reveals

Dr. Mark Witkowski from Charité University Hospital in Berlin led the analysis. In the Canadian cohort, people with high xylitol concentrations had a 57% greater risk of complications over the six-year follow-up period than those with the lowest concentrations. In the European cohort, which was observed for 30 years, the risk was 18% higher.

Xylitol is produced naturally in the body as a byproduct of glucose metabolism. However, in processed foods, drinks, and oral care products, xylitol levels can exceed natural concentrations by more than a thousand times. The link between blood xylitol and cardiovascular risk appears to follow a dose-response pattern and is independent of age, sex, diabetes, high blood pressure, or body weight.

Earlier laboratory studies indicate that xylitol may enhance blood coagulation and stimulate clot formation. Still, because the current study is observational, it cannot establish a direct cause-and-effect relationship.

Researchers are calling for urgent reassessment of xylitol safety guidelines and further trials to better understand its impact on health.

These findings highlight the need for caution when choosing sugar substitutes, as their biological effects may be more complex than previously understood. Consumers deserve clear information about potential risks and should consider alternatives in light of the latest evidence. More research will be necessary to provide firmer recommendations on xylitol and similar sweeteners in diets and medical products.
