The Social Insurance Fund of Ukraine has released a list of documents that can be submitted for digitization to confirm insurance coverage periods prior to January 1, 2004. It has also outlined what paperwork is acceptable if the original work record book is lost. For periods of employment before that date, the primary proof remains the work record book. Coverage after January 1, 2004, is calculated using data from the Insured Persons Register within the State Register of Compulsory State Social Insurance.

Documents for Digitization and in Case of a Lost Work Record Book

When submitting records for digitization, individuals must upload several supporting documents in addition to the work record book to confirm their insurance history before January 1, 2004. These include:

  • full-time education diplomas and their accompanying supplements;
  • a military ID, including all pages showing conscription, service, and discharge details;
  • a child's birth certificate;
  • documents proving a name change;
  • certificates verifying preferential or special service periods.

If the work record book is lost, the Social Insurance Fund of Ukraine has provided a specific list of alternative documents that may be used to prove employment history. These include:

  • extracts or certificates generated from information system databases;
  • personal salary accounts and payroll records;
  • identification cards and professional references;
  • written employment contracts and agreements with notes confirming their fulfillment;
  • union membership cards showing records of dues payments for the relevant period.

This document list was approved in accordance with the Procedure established by Cabinet of Ministers of Ukraine Resolution No. 637 of August 12, 1993. This information is essential for anyone looking to validate their insurance record or who has encountered the issue of a missing work record book.
