New Study Links Visceral Fat to Testosterone Decline in Men

Researchers Carl Friedl and Adam Potter from the U.S. Army Environmental Research Institute have uncovered that testosterone levels in men aged 20 to 59 are more strongly influenced by the amount of visceral fat than by age itself. Their extensive analysis, involving nearly 5,000 American men, revealed a significant association between elevated visceral fat—fat stored deep within the abdominal cavity around organs like the liver, stomach, and intestines—and lower testosterone.

The Roles of Age and Lifestyle Factors

Testosterone typically begins to decrease after the age of 35, but when visceral fat levels are taken into account, the connection between testosterone decline and age within this group weakens considerably. Prior research has also demonstrated links between obesity and reduced testosterone. For example, a 2007 study of 1,667 men showed that a 4 to 5 point rise in BMI corresponds to a testosterone drop equivalent to aging 10 years.

  • Another longitudinal study tracking 1,382 men over five years concluded that testosterone decline is not inevitable; factors such as smoking, overall health, obesity, and depression play important roles.

It’s important to note that common measurements like BMI and waist circumference don’t always accurately capture visceral fat levels. Even younger men with a normal weight can harbor significant visceral fat, which may negatively affect their health and hormone balance.

Visceral fat contributes to insulin resistance, triggers inflammation, and increases the conversion of testosterone to estradiol. Because obesity is a recognized risk factor for low testosterone, reducing visceral fat—potentially through weight loss medications—could improve testosterone levels. The study's authors emphasize evaluating metabolic causes of low testosterone before initiating hormone replacement therapy.

This research highlights the necessity of considering not only age but also visceral fat and overall health status when assessing the risks of low testosterone in men.

Given visceral fat’s strong influence on hormonal health, targeting its reduction may prove crucial for enhancing wellness in middle-aged men. Future studies are expected to clarify these relationships further and guide the development of new treatments to maintain healthy testosterone levels.
