Asthma Prevention in Children: New Review Points to Vitamin D Before Conception

How Vitamin D Supplementation May Affect Childhood Asthma Risk

Childhood asthma is a major concern for parents and clinicians, so research into prevention is especially valuable. A review in the journal Nutrients explores whether vitamin D could lower the risk of pediatric asthma. Researchers from Romania highlight the weeks before conception as a critical window for prevention, since fetal airways take shape by the 16th week of pregnancy. Earlier trials such as COPSAC and VDAART, however, have not delivered strong evidence that vitamin D changes asthma outcomes. The review's authors advocate for future studies where supplementation begins before conception and children are monitored for a longer period.

The authors note that most women do not realize they are pregnant until after this crucial fetal development stage has ended, so vitamin D supplements usually start too late. No large study has yet examined vitamin D given before conception or during the first trimester. In the COPSAC trial, children at age three had approximately a 20–25% lower risk of wheezing, but the result was not statistically significant. VDAART showed a modest early improvement in asthma symptoms that later disappeared.

Supplement Outcomes: How Vitamin D Performed in Studies

Vitamin D supplementation appeared to be more effective among mothers with adequate baseline vitamin D levels, particular genetic markers, or lower body weight. A meta-analysis found that consistently high vitamin D levels throughout pregnancy were associated with lower rates of asthma in six-year-olds. The authors caution, however, that the studies varied in supplement dosage, definitions of asthma, and how long children were followed. Blood levels of vitamin D were typically measured just once, a limitation that could influence results.

The review was written by researchers from Grigore T. Popa University of Medicine and Pharmacy and Iuliu Hațieganu University of Medicine and Pharmacy in Romania. They write:

“The trials conducted so far offer weak evidence against the periconceptional hypothesis because they were not designed to test it.”

Future research that includes vitamin D supplementation before conception, repeated measurements, and longer follow-up of children may provide more reliable information about whether this vitamin helps prevent childhood asthma.

The ongoing interest in vitamin D and asthma prevention highlights the importance of acting early in pregnancy. For now, the findings are not strong enough to confirm that vitamin D has a definite protective effect against asthma in children, but they are enough to justify additional investigation. In the future, this work could influence recommendations on nutrition and supplements for pregnant women.
