According to Міністерство Здоров'я: Ukraine has expanded access to its assessment of an individual's capacity for daily living and social adaptation. This procedure is now available not only to Ukrainian citizens but also to foreign nationals and stateless persons who are legally residing in the country. This policy change is part of broader efforts to integrate residents into society and provide necessary social services.
Required Documentation for the Assessment
To undergo the assessment, individuals must provide specific documents that confirm their legal status in Ukraine. The list of acceptable documents includes:
- Permanent residence permit
- Temporary residence permit
- Refugee certificate
- Certificate for a person in need of additional protection
- Passport from a country that has a visa-free travel agreement with Ukraine
- Military service record document - for foreigners and stateless persons who have signed a contract for military service in the Armed Forces of Ukraine, the State Special Transport Service, or the National Guard of Ukraine
For a successful application, the electronic referral must contain the applicant's last name, first name, and patronymic (if applicable) in Ukrainian, using transliteration rules. Passport details and the Taxpayer Identification Number (if available) are also required. Additionally, scanned copies of the documents proving legal stay in Ukraine must be submitted.
